How many words children have for feelings

Play therapy is a recognised form of therapy for children in which play — toys, art materials, sand, stories — is the medium the work happens through, rather than conversation. It exists because a seven-year-old cannot do what an adult does in a therapy room, which is sit in a chair and describe how they feel.

That is the short answer. The longer answer is more interesting, and includes something most articles on this subject leave out: the research evidence is real but considerably more mixed than the internet suggests.

Key Takeaways

A six-year-old can produce roughly five distinct words for emotions. An adult produces about fifteen (Grosse et al., Affective Science, 2021). Play is not a gentler substitute for talking — it is the vocabulary children actually have.

Play therapy is best established for children roughly ages 3 to 10, the age band specified both by the field’s own research centre and by an independent federal review.

The evidence is genuinely mixed. An independent federal review rates child-centred play therapy “Promising” — the lowest of its three positive tiers — and found most measured outcomes were null.

The famous “large effect” figure comes from one 2005 meta-analysis of studies published between 1953 and 2000. Later, more rigorous reviews report consistently smaller effects.

Registered Play Therapist (RPT) is a voluntary specialty credential from a private association, not a licence. Many excellent licensed child therapists use play-based methods without holding it.

Why play instead of talking?

Because the words are not there yet, and this is measurable.

Researchers asked 123 children and 27 adults to describe what people in illustrated scenarios were feeling, and counted how many distinct emotion words each produced. Four and five-year-olds managed an average of 4.4. Ten and eleven-year-olds managed 9.7. Adults managed 14.7 (Grosse, Streubel, Gunzenhauser & Saalbach, Affective Science, 2021). Emotion vocabulary roughly doubles every two years across this period, and sad and happy dominate what younger children produce.

Picture what that means in practice. A six-year-old whose parents are going through a separation may be experiencing something close to grief, divided loyalty, and fear about the future. The words available to express all three are, roughly, “sad” and “mad.” Asking that child to talk about their feelings is not a gentle approach — it is asking them to do something they are not developmentally equipped to do.

There is a second reason, from developmental psychology more broadly. Children in the primary school years reason capably about concrete, tangible things and find abstract or hypothetical reasoning much harder — a pattern developmental psychology has observed since Piaget, though the ages are approximate and children often show more competence earlier than his stages predicted. Many of the cognitive techniques adults find useful in therapy — examining a belief, weighing evidence for and against it — depend on exactly that kind of abstract reasoning, and reviews note these higher-order abstractions may not be fully available to many children until middle adolescence (Advances in Psychiatric Treatment).

This does not mean talk-based therapy is useless below age eleven. Adapted forms of CBT have a real evidence base with school-age children. The honest version is narrower: the cognitive parts of talk therapy become more fully accessible as abstract reasoning matures, and below that point, therapy for children is typically adapted so that play, art and story carry the work.

What actually happens in a session

Less than parents expect, and more than it looks like.

The therapist usually starts with an intake conversation with caregivers to understand what is going on, then spends time with the child — often simply observing how they play. In a non-directive session, the child chooses the activity and sets the pace. The therapist offers warmth and full acceptance, reflects back what they see, and sets limits only when limits are needed. In a more directive session, the therapist introduces a particular game, story or task aimed at something specific.

Those two modes are not rivals, and most practitioners move between them depending on the child and the moment. In our own child and teen work, which mode fits depends as much on the day as on the child.

Sessions typically run 30 to 50 minutes, usually weekly. Published estimates of a full course cluster around 10 to 20 sessions (Gillette Children’s Specialty Healthcare), though this varies considerably with what brought the child in.

The toys are chosen, not collected

This is the detail that most surprises parents, and it is the one that explains why a play therapy room looks the way it does.

Garry Landreth, who developed much of the modern framework, put the governing principle as: toys and materials should be selected, not collected. Each item is there for a reason, because what is available shapes what a child can express. The University of North Texas Center for Play Therapy — the field’s own research centre, and so not a disinterested party — groups them into three categories:

Category	What it is for	Typical materials
Real-life and nurturing	Expressing feelings directly; working through family roles, relationships, nurturing and control	Doll families, dollhouses, puppets, cars and trucks, play money
Acting-out and release	A safe channel for intense feeling — anger, frustration, hostility	A bop bag, toy soldiers, wild animals, egg cartons and popsicle sticks that are meant to be broken
Creative expression	Spontaneity and expressiveness, both constructive and destructive	Sand, water, paints, large paper, crayons, blocks, musical instruments

Research tracking which materials children actually reach for in child-centred play therapy found the sand tray and sand tools among the most frequently used, though use varies a good deal between children.

Does play therapy actually work?

Here is where this article will differ from most of what you will read, because the honest answer is: moderately, for some things, with real gaps in the evidence.

Much of what you will read online about play therapy’s effectiveness traces back to one source — a 2005 meta-analysis reporting a large effect. It is repeated widely enough to look like overwhelming consensus. It is one study, echoed.

And it is the weakest of the available evidence. It pooled 93 studies published between 1953 and 2000, and has been criticised for including treatments that do not meet the standard definition of play therapy, for the uneven methodological quality of the included studies, and for a lack of long-term outcome data.

When you line up the reviews in order, a familiar pattern appears.

Reported effect sizes vary widely across reviews Reported effect sizes vary widely across reviews. lollipop data: Bratton 2005 0.8; Parker 2021 0.48; Lin & Bratton 2015 0.47; Ray 2015, external. 0.34; Ray 2015, internal. 0.21; HHS federal review 0.05.Source: Published meta-analyses and the HHS Title IV-E Prevention Services Clearinghouse review of child-centered play therapy 2005-2021. The spread is the point. Estimates run from 0.80 at the top to 0.05 at the bottom, and the two lowest come from the most methodologically conservative sources in the set — a review restricted to elementary-school studies, and the independent federal assessment. The headline figure is the outlier, not the consensus.

The most sober assessment comes from the federal Title IV-E Prevention Services Clearinghouse, which reviews programmes against pre-registered quality standards and has no stake in the outcome. It rates child-centred play therapy “Promising” — the lowest of its three positive ratings. Of the studies it reviewed, most measured outcomes came back null: for behavioural and emotional functioning, 6 favourable findings against 24 null ones.

An independent systematic review screened more than 5,000 citations and found just 17 that met its inclusion criteria, in children aged 4 to 12, with effect sizes ranging from slightly negative to very large and most falling between 0.35 and 0.80 (Drisko, Corvino, Kelly & Nielson, Research on Social Work Practice, 2020). That ratio — 17 studies out of 5,000 screened — tells you something true about how thin the rigorous literature still is.

So what is it reasonably good for?

Disruptive and externalising behaviour — the strongest signal, with small-to-moderate effects reported across several reviews. One 2021 meta-analysis of 23 studies covering 908 children, median age 6, reported a moderate effect on behaviour problems.
Anxiety in younger children — promising rather than established. A controlled trial of 53 children aged 6 to 8 found significant reductions in anxiety and worry compared with an active control (Stulmaker & Ray, Children and Youth Services Review, 2015). One small trial is a reason for optimism, not a settled answer.
Internalising problems — the weakest area, and worth saying plainly. This is where effects are smallest and null findings most common.
Long-term durability — not established. Researchers consistently name the absence of long-term follow-up as a gap.

One finding worth holding onto: across this literature, involving caregivers in the work is associated with the largest effects. It rests on the weakest meta-analysis, so treat it as a reasonable signal rather than a proven law — but it points the same way as everything else we know about helping children.

What about art therapy?

Often mentioned alongside play therapy, and genuinely useful, but with a thinner evidence base.

Systematic reviews of art therapy delivered to children aged 5 to 12 in school settings report benefits across quality of life, anxiety, self-concept and emotional and behavioural difficulties, with the strongest signal in trauma and PTSD. What does not exist is a pooled effect size for art therapy with the general population of 5 to 10 year olds. The reviews are largely narrative or pilot-scale.

Children in these studies described something that matters independent of effect sizes: the arts let them express complex feelings they could not put into words. That is the same mechanism as the emotion-vocabulary finding above, arriving from a different direction.

How to vet a children’s therapist

Worth understanding before you start calling, because the credential landscape is genuinely confusing.

Registered Play Therapist (RPT) is a voluntary specialty credential awarded by the Association for Play Therapy, a private professional body. It is not a licence and not a state or government credential — it sits on top of a clinician’s existing state licence. Earning it requires a graduate mental health degree, an active independent clinical licence, substantial play therapy instruction, several hundred supervised play therapy client hours, and ongoing supervision accrued over a period of years.

Two things follow, and parents are rarely told the second one.

First, if you specifically want a clinician who has completed that formal specialisation, the Association for Play Therapy maintains a public directory, and you can verify any Ohio clinician’s underlying licence through the state licensing board.

Second — and this is the part that gets lost — a licensed therapist may legitimately use play-based methods without holding the RPT. The credential is explicitly voluntary. The ethical standard that governs any licensed clinician is their own profession’s: appropriate education, training and supervised experience for whatever they practise. Working with children through play, art and storytelling in a structured, goal-directed way is practising within a recognised and well-documented tradition.

So the useful question is not “do you have the RPT?” It is “what is your training and experience with children my child’s age, and how do you work with them?”

Frequently asked questions

What age is play therapy for?

Roughly 3 to 10 is the range specified by both the University of North Texas Center for Play Therapy and the federal clearinghouse review. Around 10 or 11, as abstract reasoning develops, approaches that involve more conversation start to fit better — though plenty of older children still benefit from having something to do with their hands.

Is my child just playing for an hour?

From the doorway, often it looks exactly like that. What is happening is that the child is choosing what to work on, and a trained adult is paying very close attention to the themes that recur, to what gets avoided, and to what changes over weeks.

How will I know if it is working?

Mostly from outside the room — at home and at school. Fewer or shorter meltdowns, better sleep, re-engaging with friends. This is one reason regular check-ins with parents matter: you have the data the therapist does not.

Will the therapist tell me what my child said?

Not verbatim, and that limit is part of what makes the room safe for a child. You should expect themes, progress and practical suggestions for home, plus a clear explanation of how confidentiality works at the first session.

How many sessions will we need?

Published estimates cluster around 10 to 20 weekly sessions, but that is an average across very different situations. A specific worry resolves faster than a response to a major loss.
